Context

Medication routines, appointments, symptoms, and care records are often managed across different people and tools. LifeLine+ brings these activities into one platform so that a patient and their care team can work from the same information.

What the platform does

Patients

Manage medications, log doses, record symptoms and mood, book appointments, and review a health timeline.

Care teams

Caregivers can monitor assigned patients and receive missed-medication alerts. Doctors can review records, manage appointments, write notes, and export health reports.

Administration

Administrators manage user roles and review platform-level statistics.

Health score

The application calculates a deterministic health score from medication adherence (40%), appointment attendance (20%), mood trend (20%), symptom frequency (10%), and logging consistency (10%).
